SOCIAL WORKER PRN

Job Description

We are currently seeking a PRN Social Worker to join our team at LifeBridge Health, located in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing a range of challenges and concerns related to institutional living, family dynamics, health issues, aging, and end-of-life care.

Responsibilities

• Conduct psychosocial assessments and provide counseling to address emotional and mental health needs.
• Assist patients and families with financial concerns and discharge planning.
• Support individuals in understanding and coping with medical diagnoses, treatments, and transitions.
• Collaborate with interdisciplinary teams to ensure comprehensive care.

Requirements

• Bachelor's degree and relevant professional licensure (LBSW/LMSW/LGPC).
• 1-3 years of experience in a clinical social work setting.
• Demonstrated knowledge of social work theory and practice.
