Job Description

We are looking for a passionate PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical services to patients and families, addressing various social and emotional concerns.

Responsibilities

• Conduct psychosocial assessments for patients and families.
• Address adjustment issues, financial concerns, family relationships, and other social challenges.
• Provide counseling on end-of-life matters and facilitate discharge planning.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience in a healthcare setting.
• Licensed as LBSW, LMSW, or LGPC.
