SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our compassionate team at LifeBridge Health. In this role, you will provide clinical social work services to patients and their families, addressing various challenges and offering support during difficult times. Responsibilities

• Conduct psychosocial assessments for patients/residents and their families.
• Address common issues such as adjustment to institutional living, financial concerns, and family relationships.
• Provide counseling on medical diagnoses, aging, powerlessness, discharge planning, and end-of-life care.

Requirements

• Bachelor's degree or equivalent with 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
• Strong knowledge of social work theory and practice.
