Job Description

We are looking for a compassionate SOCIAL WORKER PRN to join our dedicated team at LifeBridge Health in Baltimore, MD. In this role, you will provide vital clinical social work services and psychosocial assessments to patients, helping them navigate various challenges such as adjustment to institutional living, financial concerns, and family relationships.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Assist in discharge planning and support patients during end-of-life care.
• Collaborate with medical teams to address patients' emotional and social needs.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed Social Worker (LBSW/LMSW/LGPC).
