Job Description

We are currently seeking a compassionate and dedicated PRN Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families, addressing a variety of challenges and concerns.

Responsibilities

Conduct psychosocial assessments and provide counseling to patients and families.
Address issues such as adjustment to institutional living, family relationships, discharge planning, and grief counseling.
Collaborate with interdisciplinary teams to ensure comprehensive care.

Requirements

Bachelor's degree and 1-3 years of relevant experience.
Licensed as LBSW, LMSW, or LGPC.
Demonstrated knowledge in social work theory and practice.
