SOCIAL WORKER PRN

Job Description

We are looking for a passionate Social Worker PRN to join our team at LifeBridge Health. As a key member of our Allied Health department, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges they may encounter. This includes assisting with adjustment to institutional living, financial concerns, family dynamics, coping with medical situations, aging-related issues, discharge planning, and end-of-life counseling.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients/residents and families.
• Support individuals in coping with various challenges related to institutional living, financial issues, family dynamics, medical procedures, aging, and end-of-life matters.
• Collaborate with healthcare teams to ensure holistic care for patients.
• Conduct counseling sessions and develop appropriate care plans.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
• Strong interpersonal and communication skills.
