Job Description

We are looking for a compassionate and dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker PRN, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges they may face.

Responsibilities

• Offer support and counseling to individuals and families dealing with adjustment to institutional living, financial concerns, family relationships, and medical diagnoses.
• Assist with discharge planning and provide counseling on end-of-life issues.
• Collaborate with healthcare professionals to ensure comprehensive care for patients.

Requirements

• Bachelor's degree and LBSW/LMSW/LGPC licensure.
• 1-3 years of relevant experience in a healthcare setting.
• Sound understanding of social work theory and practice.
