SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and experienced Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As a PRN Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing a range of common issues such as adjustment to institutional living, financial concerns, family relationships, and end-of-life care.

Responsibilities

• Offer psychosocial support to patients and their families.
• Assess and address various challenges related to medical conditions and aging.
• Collaborate with healthcare teams to create discharge plans.
• Provide counseling and emotional support related to bereavement and loss.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong interpersonal and communication skills.
