Job Description

We are looking for a passionate PRN Social Worker to join LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services to patients and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and discharge planning. If you have 1-3 years of experience and hold a certification in social work, we encourage you to apply.

Responsibilities

• Offer psychosocial assessments and counseling to patients and families.
• Assist with discharge planning and end-of-life care.
• Handle financial concerns and facilitate family relationships.

Requirements

• Bachelor's degree with LBSW/LMSW/LGPC certification.
• 1-3 years of relevant experience in social work.
• Knowledge of clinical social work theory and practice.
