Job Description
We are looking for a compassionate PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a variety of common challenges related to institutional living, financial issues, family relationships, medical procedures, aging, discharge planning, and end-of-life counseling.
Responsibilities
- Deliver high-quality clinical social work services to patients and families.
- Conduct psychosocial assessments to identify needs and develop appropriate care plans.
- Offer counseling and support related to adjustment to medical conditions, aging, and end-of-life issues.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
Requirements
- Bachelor's degree and 1-3 years of relevant experience.
- Licensed as LBSW, LMSW, or LGPC.
- Strong understanding of social work theory and practice.
- Excellent communication and interpersonal skills.
