Job Description

We are looking for an experienced and compassionate Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a PRN Social Worker at Levindale Hospital, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Assist with discharge planning and end-of-life care.
• Address financial concerns and family relationship issues.
• Collaborate with healthcare teams to ensure comprehensive patient care.

Requirements

• Bachelor's degree with 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
• Strong knowledge of social work theory and practice.
• Compassionate and empathetic approach to patient care.
