Job Description

We are looking for a Care Manager to join our team at the Agency on Aging of South Central Connecticut. As a Care Manager, you will be responsible for assessing the health, functional, and cognitive status of clients to determine the need for home care services. You will work closely with clients, educate them about available resources, assist with program applications, and monitor the effectiveness of care plans.

Responsibilities

  • Complete annual assessments and necessary paperwork for continued services.
  • Educate clients on resources and program services.
  • Assist clients with applications for supportive services.
  • Monitor and evaluate care plans, adjusting as needed.
  • Ensure timely documentation and reports.
  • Meet critical deadlines for service delivery.

Requirements

  • Bachelor's degree in health, social work, gerontology, nursing, or related field.
  • Minimum of one year of experience working with older adults or individuals with disabilities.
  • Excellent interpersonal and organizational skills.
  • Knowledge of community resources and state-funded program documentation requirements.
  • Bilingual proficiency is a plus.
