We are currently looking for a dedicated Home Health Registered Nurse (RN) on a Per Diem basis to join our team at Interim HealthCare of West Palm Beach. Responsibilities
- Deliver in-home care to patients recovering from illnesses, injuries, and chronic conditions post-hospital or rehab discharge
- Collaborate with healthcare professionals, supervise LPNs, CNAs, and aides in patient care delivery
- Conduct patient assessments, document progress, and communicate changes to physicians
- Educate patients on treatment plans, medications, therapies, and lifestyle modifications
- Provide guidance to families on patient care, medication management, and home safety
- Ensure treatment goals are achieved and facilitate smooth patient discharge
Requirements
- Minimum 1 year of acute care experience, preferably in home healthcare
- Proficient in Oasis documentation
- Graduate of an accredited nursing program with an active RN license in Florida
- CPR certification
- Familiarity with state and federal home health regulations
- Clinical expertise in a nursing specialty, excellent communication skills, and compassionate care approach
