Primary Care Physician (Bilingual Spanish)

Job Description

We are hiring a Primary Care Physician (Bilingual Spanish) to join Esperanza Health Centers, a community health organization dedicated to providing comprehensive care to older adults through a Program of All Inclusive Care for the Elderly (PACE). As a Primary Care Physician, you will deliver primary medical care, focusing on preventive health measures and healthy lifestyle promotion for participants over 55 years of age.

Primary Duties and Responsibilities

• Manage primary care services at the PACE Center, participants' homes, and post-acute care facilities.
• Collaborate with the Interdisciplinary Team (IDT) to develop individualized care plans.
• Participate in quality improvement initiatives, such as peer reviews and care oversight.
• Maintain communication with partner facilities to ensure continuity of care.

Requirements

• Bilingual proficiency in English and Spanish is mandatory.
• Board certification in primary care; geriatrics specialization preferred.
• Valid physician license in the State of Illinois.
• Experience working with older adults is an advantage.
• Valid driver's license and current automobile insurance.
