INPATIENT CASE MANAGER (RN)

Job Description

We are looking for an Inpatient Case Manager (RN) to join our team at LifeBridge Health. This full-time position based in Randallstown, MD at Northwest Hospital involves discharge planning coordination and intervention in collaboration with the clinical team and medical providers. The role focuses on promoting patient wellness, improving care outcomes, and ensuring efficient utilization of health services for patients with complex health needs.

Responsibilities

• Coordinate discharge planning with the clinical team and medical providers.
• Promote patient wellness and improved care outcomes.
• Ensure efficient utilization of health services.
• Minimize denials of payment among patients with complex health needs.

Requirements

• Bachelor's degree with specialization in the field.
• BSN required from CCNE accredited schools.
• Prior experience in Utilization Review Nurse, Clinical Care Coordinator, Patient Care Integrator, or RN Case Manager roles.
