Job Description

Asociación Puertorriqueños en Marcha, Inc. is seeking a Post Permanency Coordinator to join our team in Philadelphia, PA. This role involves ensuring compliance with Adoption State and County regulations for children, youth, and families referred to us. The coordinator will provide case management services related to permanency to the children, permanency resource parents, and foster parents in alignment with SWAN guidelines.

Responsibilities

  • Assess the physical condition of resource homes.
  • Complete Post Permanency Services as per SWAN guidelines.
  • Document all required assessments accurately.
  • Monitor service progress and escalate issues as needed.
  • Maintain up-to-date files and provide status updates.
  • Assist with training and social work services for program families.
  • Attend meetings, conferences, and court sessions as required.

Requirements

  • Bachelor's Degree in Social Work, Psychology, or related field.
  • Minimum 1 year experience in adoption or social service delivery.
  • Knowledge of PA Chapter 3700 and Microsoft Office Suite.
