Registered Nurse (RN) Hospice Home Visits

Job Description

We are looking for a compassionate and dedicated Registered Nurse (RN) to join CareerStaff Unlimited as part of our Hospice Home Visits team in Dublin, GA. As a Registered Nurse in this role, you will have the opportunity to provide exceptional care for patients with life-limiting illnesses, working collaboratively with a team of caregivers to promote comfort and quality of life.

Responsibilities

• Demonstrate excellence in customer service and public relations to maintain a positive professional image.
• Conduct comprehensive assessments of patients, considering physical, emotional, spiritual, and psychosocial factors.
• Develop individualized care plans in collaboration with the interdisciplinary hospice team.
• Perform regular home visits to evaluate patient conditions, manage pain and symptoms, and educate patients and families on hospice care.
• Maintain accurate documentation of patient care and communication with the hospice team.

Requirements

• Minimum 12 months of hospice in-home experience within the past 3 years.
• Experience with HCHB software.
• BLS certification from the American Heart Association.
• Valid RN license in Georgia.
