Job Description

The Program Coordinator will play a crucial role in supporting community partners to address the needs of youth facing housing instability and homelessness. This position is responsible for providing direct client services tailored to individual case plans, focusing on areas such as parenting, education, employment, and housing stability. Additionally, the role involves crisis intervention, service assessment, program coordination, outreach, transportation assistance, and community referrals.

Essential Duties

  • Provide intervention and oversight for assigned clients
  • Collaborate with Advocates to deliver effective client services
  • Support clients in progressing in key areas through communication and coordination
  • Maintain facility conditions in line with case plans
  • Offer crisis intervention and guidance for daily living skills
  • Ensure compliance with program rules, regulations, and policies
  • Maintain records for each client

Requirements

Education and Experience:

  • Degree in Social Work, Psychology, Human Development, Child/Youth Development, or related field
  • Experience in serving individuals experiencing homelessness
  • Strong interpersonal and communication skills
  • Ability to collaborate effectively and maintain flexible work hours
  • Valid Wisconsin driver's license

Physical and Mental Demands:

  • Physically able to care for residents and children
  • Compliance with health and safety requirements
  • Ability to perform various physical tasks and operate equipment
  • Effective communication, problem-solving, and decision-making skills
  • Capability to handle diverse work problems and lift at least 50 lbs
