Job Description
We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. In this full-time role based in Randallstown, MD, at Northwest Hospital, you will be a crucial part of the NW Care Management team. The position requires a weekend commitment with day and evening shifts from 8:30 am to 7:00 pm.
Summary
SCHEDULE: 4/10-hour shifts weekly, 8:30 am-7 pm. JOB SUMMARY: The Inpatient Care Manager collaborates with the clinical team and medical providers to coordinate discharge planning and interventions. The goal is to enhance patient wellness, improve care outcomes, optimize health service utilization, and reduce payment denials for patients with complex health needs.
Requirements
- Basic professional knowledge equivalent to a Bachelor's degree.
- BSN required from CCNE accredited schools; exception for LBH candidates in specified roles.
