Northwest Kidney Centers is looking for a dedicated Social Worker to join our team in a full-time, variable position. As a Social Worker, you will play a vital role in providing social work intake, patient advocacy services, and coordinating social services with various stakeholders. Responsibilities

• Perform social work intake and patient advocacy services.
• Collaborate with interdisciplinary teams to assess patient social service needs and develop individualized care plans.
• Educate patients on their rights and the grievance process.
• Participate in Quality Assessment Performance Improvement activities.

Requirements

• Master of Social Work (MSW) degree.
• Current WA Social Worker License.
• Previous experience in medical social work (preferred).
• Valid Washington driver's license, reliable transportation, and car insurance.
