Job Description
We are looking for a compassionate and dedicated Hospice Social Worker to join our team at LifeBridge Health. This part-time position involves working exclusively on weekends, providing essential services to individuals in the home setting under Carroll Hospice and Carroll Hospital Center's Mission Statement and S.P.I.R.I.T. Values.
Responsibilities
- Assess psychosocial, environmental, and financial needs of patients and families.
- Develop and implement personalized care plans in collaboration with patients, families, and caregivers.
- Provide counseling and support throughout the dying and grieving process.
- Coordinate homecare referrals for necessary services.
Requirements
- Master's Degree in Social Work, LCSW or LCPC certification.
- Minimum of three years of clinical experience, with one year in a community health setting preferred.
- Professional licensure in Maryland and Pennsylvania.
