INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health's Sinai Hospital in Baltimore, MD. The role involves collaborating with the clinical team and medical providers to provide discharge planning coordination and intervention for patients with complex health needs. The ideal candidate will strive to promote patient wellness, improve care outcomes, ensure efficient health service utilization, and reduce payment denials.

Responsibilities

• Coordinate discharge planning in collaboration with clinical team and medical providers.
• Promote patient wellness and improved care outcomes.
• Efficiently utilize health services.
• Minimize denials of payment among patients with complex health needs.

Requirements

• Bachelor's degree in related field; BSN preferred, ADN required.
• Maryland Registered Nurse License.
• Case management certification required within 3-5 years of hire.
