INPATIENT CASE MANAGER - SOCIAL WORKER (LMSW)

Job Description

We are looking for a dedicated Inpatient Case Manager - Social Worker (LMSW) to join our team at LifeBridge Health. As an Inpatient Social Worker, you will collaborate with the clinical team and medical provider to provide patient and family advocacy, discharge planning coordination, and psychosocial interventions for high-risk inpatients. Your role will focus on promoting patient and family wellness, improving care outcomes, and facilitating access to hospital and community resources for patients with complex health needs.

Requirements

• Master's degree in Social Work.
• 2 years of hospital social work/community social services agencies experience preferred.
• LMSW required; LCSW/LCSW-C preferred.
• MD Social Work License per level of education.
• Effective verbal and written communication skills.

Bonus Potential

Sign-on bonus: $15,000.

Location & Schedule

Baltimore, MD at Sinai Hospital. Full-time position with a weekend commitment on the day shift from 8:00am to 4:30pm.
