Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide crucial clinical social work services and psychosocial assessments to patients and their families. Your responsibilities will include addressing various social and emotional challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments for patients and families.
- Provide counseling and support for individuals facing medical challenges.
- Assist with discharge planning and end-of-life counseling.
- Collaborate with healthcare teams to ensure holistic patient care.
Requirements
- Bachelor's degree in Social Work (BSW) required, Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
