Job Description
We are looking for an Inpatient Case Manager - Social Worker (LMSW / LCSW-C) to join our team at LifeBridge Health. In this full-time position based in Baltimore, MD, at Sinai Hospital, you will provide patient and family advocacy, discharge planning coordination, and psychosocial intervention for high-risk inpatients. Your role will focus on promoting patient and family wellness, improving care outcomes, and facilitating access to hospital and community resources for a patient population with complex health needs.
Responsibilities
- Collaborate with the clinical team and medical providers to advocate for patients and families.
- Coordinate discharge planning and provide psychosocial interventions.
- Promote patient and family wellness and improved care outcomes.
- Facilitate access to appropriate hospital and community resources.
Requirements
- Master's in Social Work required.
- LMSW required; LCSW/LCSW-C preferred.
- 3-5 years of experience in hospital social work or community social services.
- Maryland Social Work License required.
- Strong verbal and written communication skills.
