Job Description
Join LifeBridge Health as an Inpatient Case Manager (RN) on a PRN basis at Northwest Hospital in Randallstown, MD. In this role, you will collaborate with the clinical team and medical providers to facilitate discharge planning coordination and interventions for a patient population with complex health needs. The primary focus is on promoting patient wellness, improving care outcomes, and optimizing health service utilization.
Responsibilities
- Coordinate with the clinical team and medical providers to develop and implement discharge plans.
- Promote patient wellness and optimize care outcomes.
- Facilitate efficient utilization of health services.
- Ensure compliance with regulatory requirements and quality standards.
Requirements
- Bachelor's degree in Nursing (BSN preferred).
- Maryland Registered Nurse License.
- American Heart Association CPR Certification.
- 3-5 years of clinical experience.
- Case Management certification required within 3-5 years of hire.
