INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a motivated Inpatient Case Manager (RN) to join our team at LifeBridge Health. In this part-time role based in Baltimore, MD at Sinai Hospital, you will work collaboratively with the clinical team and medical providers to facilitate discharge planning, intervention, and coordination for patients with complex health needs. Your focus will be on promoting patient wellness, improving care outcomes, ensuring efficient health service utilization, and reducing payment denials. Requirements

• Bachelor's degree or equivalent professional knowledge.
• BSN preferred; ADN required.
• 3-5 years of related experience in healthcare.
