Sunrise Senior Living is seeking a compassionate Home Health Aid to join our team and provide high-quality care to our residents in assisted living and reminiscence neighborhoods. As a Care Manager, you will build meaningful relationships with seniors and their families, assist with daily activities, and contribute to their overall well-being.

Responsibilities

• Provide personalized care and assistance to assigned group of residents.
• Notify management of any changes in residents' conditions.
• Participate in the development of Individualized Service Plans and ensure adherence to safety regulations.
• Engage residents in life skills and enriching activities.

Qualifications

• Dedication to serving seniors with excellent customer service skills.
• Positive attitude and ability to work in a team environment.
• High School diploma/GED; certifications may be required per state regulations.
• Previous experience working with seniors preferred.
• Good organizational, time management, and communication skills.
