Job Description
We are seeking a dedicated and experienced Complex Care Social Worker (LMSW) to join our team at LifeBridge Health. As a Complex Care Social Worker, you will play a crucial role in facilitating discharge planning for difficult-to-place, high-cost, and escalated patients. Your responsibilities will include implementing a multi-step escalation protocol, collaborating with the interdisciplinary team, and ensuring timely and cost-effective discharges.
Responsibilities
- Facilitate discharge planning in line with length of stay goals for complex patients.
- Identify barriers preventing effective discharges and implement escalation protocols.
- Assume responsibility for escalated patients and collaborate with the interdisciplinary team.
- Monitor difficult-to-place and high-cost patients for early intervention opportunities.
Requirements
- Master's degree in Social Work.
- Licensed Master Social Worker (LMSW) certification.
- Seasoned professional knowledge in healthcare and discharge planning.
