Job Description

We are looking for a passionate Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various common issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address common problems faced by patients/residents and families.
• Assist with adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Requirements

• Bachelor's in Social Work (BSW), Master's degree preferred.
• LCSW-C preferred.
• LGSW certification required.
