Job Description
We are currently looking for a dedicated and compassionate Social Worker to join our team at LifeBridge Health. As a Social Worker at Levindale Hospital in Baltimore, MD, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing various common challenges such as adjustment to institutional living, financial concerns, family relationships, and more.
Responsibilities
- Provide clinical social work services and psychosocial assessments.
- Address common problems related to patient and family adjustment.
- Assist with discharge planning and counseling.
- Collaborate with interdisciplinary teams to support patients' well-being.
Requirements
- Bachelor's in Social Work (BSW), Master's degree preferred.
- LCSW-C preferred, LGSW certification required.
- Experience in hospital social work and knowledge of relevant state regulations.
- Strong communication, empathy, and organizational skills.
