Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a full-time Social Worker, you will be responsible for providing clinical social work services and psychosocial assessments to patients, residents, and families. Your duties will include addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, discharge planning, and counseling related to death and dying.
Responsibilities
- Provide clinical social work services to patients/residents and families.
- Conduct psychosocial assessments to address common challenges.
- Offer counseling and support for issues related to aging, medical diagnoses, and discharge planning.
Requirements
- Bachelor's in Social Work (BSW); Master's degree preferred.
- LCSW-C preferred; LGSW certification required.
- Prior experience in a healthcare setting is a plus.
