Responsibilities
- Conduct psychosocial assessments for patients/residents and their families.
- Provide counseling and support for various issues including discharge planning and bereavement.
- Coordinate with healthcare teams to enhance patient well-being.
Requirements
- Bachelor's degree in Social Work (BSW) required; Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
