Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families. Your responsibilities will include addressing various issues such as adjustment to institutional living, financial concerns, family relationships, aging-related changes, discharge planning, and counseling for end-of-life matters.
Requirements
- Bachelor's in Social Work (BSW) required.
- Master's degree in Social Work preferred.
- LCSW-C certification preferred.
- LGSW certification required.
