Job Description
We are looking for a dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide crucial clinical social work services to patients/residents and families, assisting with various challenges and transitions they may face in a healthcare setting.
Responsibilities
- Conduct psychosocial assessments for patients and their families.
- Provide counseling and support for issues related to institutional living, financial concerns, family relationships, and more.
- Assist in discharge planning and end-of-life counseling.
Requirements
- Bachelor's in Social Work (BSW) required.
- Master's degree in Social Work preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) highly desirable.
- Licensed Graduate Social Worker (LGSW) certification.
