Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, addressing various challenges they may encounter. Your role will involve assisting patients and families with a wide range of issues such as adjustment difficulties, financial concerns, family relationships, aging-related changes, and end-of-life counseling.
Key Responsibilities
- Conduct psychosocial assessments and provide clinical social work services.
- Assist patients and families with coping strategies and mental health support.
- Address financial concerns and facilitate access to resources.
- Participate in discharge planning and end-of-life counseling.
Requirements
- Bachelor's degree in Social Work (BSW) required.
- Master's degree in Social Work preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) certification preferred.
- Licensed Graduate Social Worker (LGSW) certification.
