Job Description
LifeBridge Health is currently seeking a compassionate Social Worker to join our dynamic team at Levindale Hospital in Baltimore, MD. As a full-time Social Worker, you will have the opportunity to provide clinical social work services and psychosocial assessments to patients/residents and their families, addressing various challenges and providing support in critical areas.
Responsibilities
- Offer clinical social work services and psychosocial assessments.
- Address common issues such as adjustment to institutional living, financial concerns, family relationships, and more.
- Provide counseling on complex issues related to aging, powerlessness, and end-of-life care.
- Collaborate with healthcare teams to support discharge planning and ensure holistic care.
Requirements
- Bachelor's in Social Work (BSW); Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification.
