INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated and qualified Inpatient Case Manager (RN) to join our team at LifeBridge Health. As an Inpatient Care Manager, you will collaborate with the clinical team and medical providers to coordinate discharge planning and interventions for patients with complex health needs. Your role is crucial in promoting patient wellness, improving care outcomes, and ensuring efficient utilization of health services.

Responsibilities

• Coordinate discharge planning activities in collaboration with the clinical team.
• Promote patient wellness and efficient utilization of health services.
• Minimize denials of payment by ensuring compliance with care plans.
• Provide support and guidance to patients and their families.

Requirements

• Bachelor's degree in nursing (BSN) from CCNE accredited schools.
• Professional knowledge and experience in a specialized field.
• Experience in care coordination and discharge planning is preferred.
